Indications

Enaril is indicated for: - Hypertension: All grades of essential hypertension and renovascular hypertension. - Heart failure: As adjunctive therapy with non-potassium-sparing diuretics and where appropriate, digitalis. - Severe heart failure: Improves symptoms and reduces mortality and hospitalization. - Mild to moderate heart failure: Improves symptoms and reduces mortality and hospitalization.

Pharmacology

Enalapril, after hydrolysis to enalaprilate, inhibits Angiotensin Converting Enzyme (ACE). ACE is a peptidyl dipeptidase that catalyses the conversion of angiotensin I to the vasoconstrictor angiotensin II. Angiotensin II also stimulates aldosterone secretion by the adrenal cortex. The beneficial effects in hypertension and heart failure result primarily from suppression of the renin-angiotensin aldosterone system.

Dosage & Administration

Essential and renovascular hypertension: Starting dose 5 mg once daily. With concomitant diuretic therapy: recommended initial dose is 2.5 mg. Usual maintenance dose is 10-20 mg once daily. Maximum 40 mg daily. Heart failure: Initiate with 2.5 mg once daily under close medical supervision. Titrate gradually to maintenance dose of 10-20 mg. Elderly: Starting dose 2.5 mg. Renal impairment: Starting dose 2.5 mg, titrate carefully.

Side Effects

Severe hypotension, renal failure, dizziness, headache, fatigue, orthostatic hypotension, syncope, nausea, diarrhoea, muscle cramps, rash, persistent dry cough. Less common: myocardial infarction, pancreatitis, hepatitis, angioedema, Stevens-Johnson syndrome, impotence, alopecia, tinnitus.

Precautions & Warnings

Monitor renal function before and during therapy. Symptomatic hypotension most likely in patients with severe heart failure on high doses of loop diuretics. Initiate with small dose (2.5 mg) to lessen duration of any hypotensive effect. Cough has been reported; resolves after discontinuation. In haemodialysis patients using high flux membranes: high incidence of anaphylactoid reactions — avoid concomitant use.

Contraindications

Hypersensitivity to the product or any component. Patients with history of angioneurotic oedema relating to previous ACE inhibitor treatment. Contraindicated during pregnancy.

Pregnancy & Lactation

Contraindicated in pregnancy; shown to be fetotoxic. Enalapril and enalaprilat are excreted in human milk; caution should be exercised in lactating mothers.

Storage Conditions

Do not store above 25°C. Keep away from light and wet place. Keep out of reach of children.
