DESCRIPTION
Edemide is a combination of a loop diuretic, Furosemide and a potassium sparing diuretic, Spironolactone. This combination produces synergistic or additive diuretic effects. Furosemide inhibits the Na+/K+/2CI- cotransporter in the ascending limb of Loop of Henle and blocks the re-absorption of Na+
, K+ and Clions. Spironolactone inhibits the reabsorption of sodium and excretion of potassium at the distal tubule by blocking the action of aldosterone. So the excretion of sodium is increased and the excess loss of potassium, induced by the Furosemide, is decreased.
COMPOSITION
EDEMIDE : Each film-coated tablet contains Spironolactone BP 50 mg and Furosemide BP 20 mg.
EDEMIDE 40 : Each film-coated tablet contains Spironolactone BP 50 mg and Furosemide BP 40 mg.

DOSAGE AND ADMINISTRATION
EDEMIDE : 1 to 4 tablets daily according to the patient’s response.
EDEMIDE 40 : For previously stabilised patients requiring higher dosage of spironolactone and furosemide EDEMIDE 40 tablet can be used at a dose of one to two tablets daily (spironolactone 50 to 100 mg and furosemide 40 to 80 mg.

SIDE EFFECT
Fatigue, skin rashes, diarrhea, constipation, nausea, vomiting, abdominal pain, hyperglycemia, hypotension, gynecomastia, irregular menstrual cycle, impotence may occur.
CONTRAINDICATION
Edemide is contraindicated in patients with a history of hypersensitivity to Furosemide or Spironolactone. It is also contraindicated in acute renal insufficiency, anuria and hyperkalemia.
PRECAUTION
It is recommended that individuals taking this medicine have their fluid and salt (electrolyte) balance monitored regularly.
Edemide should be used with caution in diabetes, enlarged prostate, hypotension, hypovolemia.
USE IN PREGNANCY AND LACTATION
Edemide should be used with caution during pregnancy.
Edemide passes into breast milk. It is recommended that mothers should avoid using this medicine while breast feeding.
USE IN CHILDREN AND ELDERLY
Edemide is not suitable for use in children.
Furosemide and Spironolactone may both be excreted more slowly in the elderly.
DRUG INTERACTION
When taken together with ACE inhibitors or potassium salts there is an increased risk of hyperkalemia.
Spironolactone increases the levels of cardiac glycosides such as digoxin in the blood and this may result in digitalis toxicity.
Corticosteroids may cause hypokalemia if they are used with Spironolactone.
The blood pressure lowering and diuretic effects of Furosemide may be reduced or abolished when used together with indometacin and possibly other non-steroidal anti-inflammatory drugs (NSAIDs).
Furosemide may increase the ototoxicity of aminoglycoside antibiotics.
Simultaneous administration of sucralfate and Furosemide may reduce the natriuretic and anti-hypertensive effect of Furosemide.
