Overview

Efaxim 200 mg Tablet contains Rifaximin, a broad-spectrum non-systemic antibiotic derived from rifamycin, manufactured by Square Pharmaceuticals PLC. Rifaximin acts locally in the gut, inhibiting bacterial RNA synthesis, with negligible systemic absorption. It is used for travellers' diarrhoea caused by non-invasive strains of E. coli, hepatic encephalopathy prophylaxis and treatment, irritable bowel syndrome with predominant diarrhoea (IBS-D), and small intestinal bacterial overgrowth (SIBO).

Dosage and Administration

Travellers' diarrhoea: 200 mg three times daily for 3 days. Hepatic encephalopathy: 400 mg three times daily as part of combination management. SIBO/IBS-D: as directed by physician. Take with or without food.

Interaction

Due to minimal systemic absorption, clinically significant interactions are rare. Theoretical risk of interaction with drugs also metabolised via P-gp if gut absorption occurs. Cyclosporin levels may increase in patients with advanced liver disease.

Contraindications

Contraindicated in diarrhoea complicated by fever or blood in stools (possible invasive enteropathogens — rifaximin is ineffective in systemic infections), and known hypersensitivity to rifaximin or rifamycin antibiotics.

Side Effects

Well tolerated. Occasional flatulence, headache, abdominal pain, and nausea. C. difficile-associated colitis is rare given minimal systemic absorption.

Pregnancy and Lactation

Limited safety data in pregnancy — use only if clearly needed. Not recommended during breastfeeding.

Precautions and Warnings

Not for use in diarrhoea with fever or blood — refer to physician promptly. Ensure adequate hydration. If symptoms do not improve within 24–48 hours, seek medical advice.
