Overview
Fusid Tablet (Furosemide 40 mg) is a potent loop diuretic used in the management of oedema and hypertension. Furosemide acts on the ascending limb of the loop of Henle by inhibiting the Na⁺-K⁺-2Cl⁻ co-transporter, producing a rapid, dose-dependent diuresis with accompanying excretion of sodium, chloride, potassium, and other electrolytes. It is indicated for oedema associated with congestive heart failure, renal impairment, nephrotic syndrome, and hepatic cirrhosis, as well as mild to moderate hypertension.
Dosage and Administration
Adults — Oedema: Initial dose of 40 mg daily, taken in the morning. The dose may be increased to 80 mg daily or more depending on response. Hypertension: 80 mg daily in divided doses, adjusted to response, used in combination with other antihypertensives. Tablets should be taken orally, with or without food. To minimise sleep disturbance from diuresis, do not take late in the day.
Interaction
ACE inhibitors and ARBs may cause profound hypotension when started in patients already on furosemide — temporarily reduce or hold furosemide before initiation. Furosemide elevates serum lithium levels, increasing toxicity risk — monitor levels closely. Combined use with aminoglycosides or cisplatin increases ototoxicity and nephrotoxicity risk. NSAIDs reduce the diuretic response. Furosemide-induced hypokalaemia potentiates digoxin toxicity.
Contraindications
Contraindicated in anuria, states of severe electrolyte depletion (hypokalaemia, hyponatraemia) or hypovolaemia without hypertension, hypersensitivity to furosemide or sulphonamide derivatives, and hepatic pre-coma or coma associated with liver cirrhosis. Caution is required in patients with gout, diabetes, or prostatic enlargement.
Side Effects
Electrolyte disturbances (hypokalaemia, hyponatraemia, hypomagnesaemia) are common, particularly with high doses or prolonged use. Other effects include dehydration, postural hypotension, muscle cramps, hyperuricaemia (with risk of gout), and transient increases in blood glucose. Ototoxicity (tinnitus, hearing impairment) may occur, especially with rapid IV use or high oral doses. Rare: blood dyscrasias, interstitial nephritis, allergic skin reactions.
Pregnancy and Lactation
Furosemide crosses the placenta and is excreted in breast milk. Use during pregnancy only when clearly necessary, avoiding the first trimester where possible. Furosemide inhibits lactation and is present in breast milk; breastfeeding is generally not recommended during treatment. Consult a physician before use.
Precautions and Warnings
Monitor serum electrolytes (particularly potassium), urea, and creatinine regularly, especially in elderly patients, those with renal impairment, or on prolonged high-dose therapy.
