Overview

Gabastar 100 mg (Gabapentin) is a structural analogue of gamma-aminobutyric acid (GABA) with anticonvulsant and analgesic properties. Although its exact mechanism is not fully understood, gabapentin binds to the alpha-2-delta subunit of voltage-gated calcium channels, reducing the release of excitatory neurotransmitters. It is indicated as adjunctive therapy for partial seizures (with or without secondary generalisation) in adults and children over 3 years, and for the management of neuropathic pain including post-herpetic neuralgia and painful diabetic neuropathy.

Dosage and Administration

Neuropathic pain (adults): Day 1 — 300 mg once; Day 2 — 300 mg twice daily; Day 3 — 300 mg three times daily. Titrate as needed up to 1800 mg/day in three divided doses. Epilepsy (adults and children over 12 years): Start at 300 mg three times daily; effective range 900–1800 mg/day in three divided doses; maximum 3600 mg/day. Children 3–12 years: 10–15 mg/kg/day in three divided doses, titrating over 3 days. Dose reduction required in renal impairment. Gabastar 100 mg is particularly useful for initial dose titration. Tablets may be taken with or without food.

Interaction

Antacids containing aluminium or magnesium reduce gabapentin absorption by approximately 20% — take gabapentin at least 2 hours after antacids. CNS depressants (alcohol, opioids, benzodiazepines) have additive sedative effects. Morphine may increase gabapentin plasma levels by up to 44%. Hydrocodone and other opioids increase risk of respiratory depression when combined with gabapentin.

Contraindications

Contraindicated in patients with known hypersensitivity to gabapentin or any component of the formulation. Not recommended as monotherapy for generalised seizures. Rare but serious: suicidal ideation, anaphylaxis, angioedema, and severe respiratory depression (especially with CNS depressants or at high doses).

Pregnancy and Lactation

Gabapentin crosses the placenta and is present in breast milk. Use during pregnancy only if the benefit clearly outweighs the risk — animal studies have shown fetal toxicity at high doses. Neonates exposed in utero should be monitored for withdrawal symptoms. Breastfed infants should be monitored for sedation; an informed decision should be made whether to discontinue breastfeeding or the drug.

Precautions and Warnings

Antiepileptic drugs including gabapentin increase the risk of suicidal thoughts and behaviour — monitor patients closely. Do not abruptly discontinue gabapentin, as this may trigger rebound seizures; taper the dose gradually over at least one week. Use with caution in elderly patients who may be more sensitive to CNS effects and respiratory depression. Dose adjustment is required in renal impairment (based on creatinine clearance).
