Overview

Gutfix 24 mcg Capsule contains Lubiprostone, a locally acting chloride channel activator that enhances chloride-rich intestinal fluid secretion without altering serum sodium or potassium concentrations. Lubiprostone specifically activates ClC-2 chloride channels — normal constituents of the apical membrane of the human intestine — increasing intestinal motility and facilitating the passage of stool. It also restores mucosal barrier function by recovering tight junction protein complexes. Gutfix 24 mcg is indicated for the treatment of Chronic Idiopathic Constipation in adults, Opioid-Induced Constipation in adults, and is FDA-approved for the treatment of Irritable Bowel Syndrome with Constipation (IBS-C) in women over 18 years of age. Relief of symptoms typically occurs within 24 hours of administration.

Dosage and Administration

Chronic Idiopathic Constipation: Lubiprostone 24 mcg taken twice daily orally with food and water.

Opioid-Induced Constipation: Lubiprostone 24 mcg taken twice daily orally with food and water.

Irritable Bowel Syndrome with Constipation: Use the 8 mcg strength twice daily (not this 24 mcg formulation).

Patients with moderate to severe hepatic impairment should use a reduced dosage as directed by their physician. Safety and effectiveness in children and adolescents have not been established.

Interaction

Based on in vitro human microsome studies, there is a low likelihood of clinically significant drug–drug interactions with Gutfix. Cytochrome P450 isoenzymes are not involved in the metabolism of Lubiprostone, and no protein binding-mediated drug interactions of clinical significance are anticipated.

Contraindications

Gutfix is contraindicated in patients with known or suspected mechanical gastrointestinal obstruction. Patients presenting with symptoms suggestive of bowel obstruction should be evaluated before initiating therapy.

Side Effects

Common side effects include nausea, vomiting, loose stools, dry mouth, stomach discomfort, headache, and dizziness. Nausea may be reduced by taking Gutfix with food. Gutfix should not be prescribed to patients with severe diarrhoea. Adverse effects from overdose may include nausea, diarrhoea, vomiting, dizziness, headache, and abdominal pain.

Pregnancy and Lactation

Pregnancy Category C. Lubiprostone should be used during pregnancy only if the potential benefits justify the potential risk to the foetus. It is not known whether Lubiprostone is excreted in human milk; caution is advised when administering to breastfeeding women.

Precautions and Warnings

Take Gutfix with food and water to minimise nausea. Do not use in patients with severe diarrhoea. Before initiating therapy in patients with symptoms suggestive of mechanical gastrointestinal obstruction, confirm that no obstruction exists. Reduce the dose in patients with moderate to severe hepatic impairment. Not studied in patients with renal impairment. Safety and efficacy in children and adolescents have not been established.
