Overview

Halobet 0.05% Cream contains Halobetasol Propionate, a super-high potency topical corticosteroid indicated for the relief of inflammatory and pruritic manifestations of corticosteroid-responsive dermatoses. Like other topical corticosteroids, Halobetasol Propionate exerts anti-inflammatory, antipruritic, and vasoconstrictive actions. The mechanism of anti-inflammatory activity involves the induction of phospholipase A2 inhibitory proteins (lipocortins), which control the biosynthesis of potent inflammatory mediators such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid. Due to its super-high potency, treatment should be limited to two consecutive weeks, and total dosage must not exceed 50 gm per week to avoid suppression of the Hypothalamic-Pituitary-Adrenal (HPA) axis. Use in children under 12 years is not recommended.

Dosage and Administration

Apply a thin layer of Halobet 0.05% Cream to the affected skin area once or twice daily as directed by the physician, and rub in gently and completely. Due to its super-high potency classification, treatment should not exceed two consecutive weeks and the total amount used should not exceed 50 gm per week. Discontinue therapy once control is achieved. If no improvement is seen within 2 weeks, reassessment of diagnosis may be necessary. Do not use with occlusive dressings.

Interaction

No significant drug–drug interactions are expected with topical Halobetasol Propionate under normal conditions of use. However, concomitant use of other topical corticosteroids on the same area may increase the risk of systemic side effects. Avoid concurrent use of other immunosuppressants or corticosteroids without physician supervision.

Contraindications

Halobet Cream is contraindicated in patients with a history of hypersensitivity to Halobetasol Propionate or to any component of the formulation. It should not be used on the face, groin, or axillae. Not intended for ophthalmic, oral, or intravaginal use.

Side Effects

Infrequently reported adverse effects with topical corticosteroids include burning, itching, dryness, folliculitis, acne, hypopigmentation, perioral dermatitis, allergic contact dermatitis, skin atrophy, secondary infections, striae, and miliaria. Systemic effects — including HPA axis suppression, Cushing's syndrome, hyperglycaemia, and glucosuria — may occur with excessive use, prolonged application, or large-area application.

Pregnancy and Lactation

Topical corticosteroids should be used during pregnancy only if the potential benefit justifies the potential risk to the foetus. It is not known whether topical administration results in sufficient systemic absorption to produce detectable quantities in breast milk. Caution should be exercised when administering to breastfeeding women.

Precautions and Warnings

Systemic absorption may cause reversible HPA axis suppression, Cushing's syndrome, hyperglycaemia, and glucosuria. Patients using large amounts, applying to large areas, or using under occlusive dressings should be monitored periodically with urinary free cortisol and ACTH stimulation tests. If HPA axis suppression is noted, reduce frequency, withdraw the drug, or substitute a less potent corticosteroid. Use in children under 12 years is not recommended, as paediatric patients are at greater risk of HPA axis suppression.
