Overview

Ambrisan contains Ambrisentan, manufactured by Square Pharmaceuticals PLC. It is an endothelin receptor antagonist (ERA) selective for the endothelin type-A (ETA) receptor, indicated for the treatment of pulmonary arterial hypertension (PAH) to improve exercise capacity and delay clinical worsening.

Dosage and Administration

The recommended starting dose is 5 mg once daily, which may be increased to 10 mg once daily depending on response and tolerability, as advised by the physician. May be taken with or without food.

Interaction

Cyclosporine A increases ambrisentan exposure; co-administration should be limited to 5 mg once daily. Caution advised with rifampin and other strong inducers of drug metabolism.

Contraindications

Contraindicated in pregnancy and in patients with known hypersensitivity to ambrisentan or any component of the formulation. Also contraindicated in idiopathic pulmonary fibrosis.

Side Effects

Peripheral oedema, headache, nasal congestion, flushing, and palpitations. Liver function abnormalities may rarely occur.

Pregnancy and Lactation

Contraindicated in pregnancy due to risk of fetal harm; effective contraception is required in females of reproductive potential. Not recommended during breastfeeding.

Precautions and Warnings

Monitor for signs of fluid retention and hepatic impairment. Pregnancy test is required before initiation and monthly during treatment in women of childbearing potential.
