Overview

Clotinex 4000 Anti-Xa IU Injection contains Enoxaparin Sodium, a low molecular weight heparin (LMWH) anticoagulant, manufactured by Square Pharmaceuticals PLC. It is used for the prevention and treatment of deep vein thrombosis (DVT), pulmonary embolism (PE), unstable angina, non-Q-wave myocardial infarction, and venous thromboembolism prophylaxis in surgical and medical patients.

Dosage and Administration

Prophylaxis: 40 mg (4000 IU) subcutaneously once daily for 7–10 days or until the patient is fully ambulant. Treatment doses are higher; administer by deep SC injection into the abdominal fat only. Rotate injection sites.

Interaction

Increased bleeding risk with anticoagulants, antiplatelet agents, and thrombolytics. NSAIDs may potentiate bleeding. Dextran solutions may interact. Avoid combining with other anticoagulants without monitoring.

Contraindications

Contraindicated in active major bleeding, thrombocytopenia (HIT), hypersensitivity to enoxaparin or heparin, and in patients with prosthetic heart valves.

Side Effects

Bleeding at injection site, haematoma, thrombocytopenia, elevated liver enzymes, and rarely heparin-induced thrombocytopenia (HIT). Do not give by IV injection.

Pregnancy and Lactation

May be used during pregnancy for thromboprophylaxis; preferred LMWH over warfarin in pregnancy. Use with caution near delivery. Transfer to breast milk is negligible.

Precautions and Warnings

Monitor platelet count regularly. Use with extreme caution in patients with renal impairment (reduce dose if CrCl <30 ml/min). Do not remove air bubble from pre-filled syringe before injection. Administer SC only.

Storage Conditions

Store at room temperature below 25°C. Do not freeze. Keep out of the reach of children.
