What is Amlotab?

Amlotab is a dihydropyridine calcium-channel blocker (Amlodipine Besilate) manufactured by Incepta Pharmaceuticals Ltd., used in the treatment of hypertension and angina pectoris. It works by causing vasodilation, reducing peripheral resistance, blood pressure, and cardiac workload.

Indications

Essential hypertension: efficacious as monotherapy, may be combined with other antihypertensives.
Angina pectoris: for chronic stable angina, as monotherapy or combination.
Vasospastic angina: for confirmed or suspected vasospastic angina.

Pharmacology

Amlodipine is a long-acting dihydropyridine calcium-channel blocker. It results primarily in vasodilation, with reduced peripheral resistance, blood pressure and afterload, increased coronary blood flow and a reflex increase in coronary heart rate, increasing myocardial oxygen supply and cardiac output.

Dosage & Administration

Hypertension: usual dose 5 mg once daily, maximum 10 mg once daily. Elderly or hepatic-impaired patients may start at 2.5 mg once daily.
Angina: 5 to 10 mg once daily, lower dose for elderly/hepatic impairment, most patients require 10 mg. May be taken without regard to meals.

Interaction

Caution with beta-blockers in patients with impaired left ventricular function. No significant interaction demonstrated with digoxin, cimetidine, or warfarin. Food does not affect absorption.

Contraindications

Hypersensitivity to dihydropyridine derivatives. Use in pregnancy should be avoided unless necessary.

Side Effects

Dizziness, flushing, headache, hypotension, peripheral edema, GI disturbances, increased micturition frequency, lethargy, eye pain, mental depression.

Pregnancy & Lactation

Pregnancy Category C — use only if benefit outweighs risk. Not known if excreted in breast milk; nursing discontinuation is recommended during use.

Precautions & Warnings

Use with caution in hepatic impairment; start at lowest dose (2.5 mg) in severe hepatic impairment, titrating slowly.
