What is Budemet?

Budemet is a combination inhaler containing Budesonide (an inhaled corticosteroid) and Formoterol Fumarate (a long-acting beta2-agonist), used for the maintenance treatment of asthma and COPD. It is intended for oral inhalation via dry powder inhaler only and must not be swallowed.

Indications

Asthma: Maintenance treatment of asthma in patients 12 years of age and older (not indicated for relief of acute bronchospasm).

COPD: Twice-daily maintenance treatment of airflow obstruction in patients with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema.

Pharmacology

Budesonide is a glucocorticosteroid with potent anti-inflammatory activity. It inhibits multiple cell types (mast cells, eosinophils, neutrophils, macrophages, lymphocytes) and mediators (histamine, leukotrienes, cytokines) involved in allergic and non-allergic airway inflammation.

Formoterol Fumarate is a selective long-acting beta2-adrenoceptor agonist. It stimulates adenyl cyclase, increasing intracellular cAMP, which causes bronchial smooth muscle relaxation and inhibits release of hypersensitivity mediators from mast cells. Onset of bronchodilation is within 1-3 minutes and duration is at least 12 hours after a single dose.

Dosage & Administration

Asthma — Adults and adolescents 12 years and older: Maintenance dose: 1 inhalation capsule twice daily. Reliever dose: 1 additional capsule as needed (not more than 6 capsules per occasion; total daily dose up to 12 capsules temporarily if needed).

Asthma — Children 4 years and older: Maintenance dose: 1 capsule (100 mcg strength) once daily. Reliever dose: 1 additional capsule as needed (not more than 4 per occasion).

COPD — Adults 40 years and older: 2 capsules (200 mcg strength) twice daily, maximum 4 capsules/day. Use with appropriate dry powder inhaler device only.

Interaction

Strong CYP3A4 inhibitors (e.g. ritonavir) should be avoided due to increased systemic corticosteroid effects. Extreme caution with MAO inhibitors and tricyclic antidepressants (potentiation of formoterol cardiovascular effects). Beta-blockers should be used with caution as they may block bronchodilatory effects and cause severe bronchospasm. Non-potassium-sparing diuretics may worsen hypokalemia caused by beta-agonists.

Contraindications

Contraindicated as primary treatment for status asthmaticus or acute episodes of asthma/COPD requiring intensive measures. Contraindicated in patients with hypersensitivity to any ingredient.

Side Effects

Formoterol-related: Tremor, palpitations, headache (common); cardiac arrhythmias, muscle cramps, hypersensitivity reactions (uncommon/rare). Budesonide-related: Hoarseness, oropharyngeal candidiasis, throat irritation; cutaneous hypersensitivity reactions, respiratory infections, growth effects in children, glaucoma, cataract (rare).

Pregnancy & Lactation

No clinical data on combined use in pregnancy. Budesonide is excreted in breast milk but at therapeutic doses no effects on the suckling child are anticipated. Formoterol in breast milk is not established in humans. Administer only when expected benefit to the mother outweighs any possible risk to the child.

Precautions & Warnings

Use with caution in diabetes mellitus, thyrotoxicosis, untreated hypokalaemia, severe hypertension, ischaemic heart disease, tachycardia, severe heart failure, glaucoma, cataract, and hypercorticism. If paradoxical bronchospasm occurs (immediate increase in wheezing after dosing), discontinue medication immediately and initiate alternative therapy. Monitor height in children on prolonged inhaled corticosteroid therapy.

Use in Special Populations

Children receiving prolonged inhaled corticosteroid treatment should have their height monitored regularly. If growth is slowed, reduce the dose and reassess. Refer to a paediatric respiratory specialist if needed.

Overdose Effects

Formoterol overdose: tremor, headache, palpitations, tachycardia, hyperglycaemia, hypokalaemia, arrhythmia, nausea, vomiting — supportive and symptomatic treatment. Acute Budesonide overdose is not expected to be clinically significant; chronic excess may cause systemic glucocorticoid effects (hypercorticism, adrenal suppression).

Storage Conditions

Store at or below 25°C, away from light and moisture. Remove inhalation capsule from blister only immediately before use. Inhalation capsules must not be swallowed. Keep out of reach of children.
