Carvista 25 mg Tablet

What is Carvista?

Carvista is a non-selective beta-blocker (Carvedilol) manufactured by Incepta Pharmaceuticals Ltd., used in the management of hypertension, angina pectoris, and mild to severe congestive heart failure. It works by blocking beta-1, beta-2, and alpha-1 adrenergic receptors, reducing heart rate, cardiac workload, and peripheral vascular resistance.

Indications

Carvista is indicated for the treatment of mild, moderate or severe heart failure of ischemic or cardiomyopathic origin, in conjunction with digitalis, diuretics and ACE inhibitor. It is also used in hypertension and angina pectoris.

Pharmacology

Carvedilol is a cardiovascular drug whose main pharmacological action is non-selective antagonism of beta-adrenergic receptors but which also possesses appreciable alpha-adrenergic antagonistic activity. It also has antiproliferative properties and is a scavenger of reactive free oxidant radicals.

Dosage & Administration

Hypertension: initially 12.5 mg once daily, increased after 2 days to 25 mg once daily; may be further increased at intervals of at least 2 weeks to a maximum of 50 mg daily.
Angina pectoris: 12.5 mg twice daily for the first 2 days, then 25 mg twice daily.
Heart failure: initially 3.125 mg twice daily (with food), increased at intervals of at least 2 weeks to 6.25 mg, then 12.5 mg, then 25 mg twice daily, to the highest tolerated dose.

Interaction

Increased monitoring of digoxin is recommended when initiating, adjusting, or discontinuing Carvista. Rifampin pretreatment causes a 60% decrease in Cmax and AUC. Beta-blockers potentiate the pressor reaction following sudden clonidine withdrawal.

Contraindications

Contraindicated in decompensated heart failure requiring IV inotropic therapy, bronchial asthma or related bronchospastic conditions, second/third-degree AV block, sick sinus syndrome (without pacemaker), cardiogenic shock, or severe bradycardia.

Side Effects

Postural hypotension, dizziness, headache, fatigue, GI disturbances, bradycardia; occasionally peripheral oedema, dry mouth, disturbed vision, impotence, influenza-like symptoms.

Pregnancy & Lactation

Should not be used during breast-feeding. Not recommended during pregnancy as no adequate studies have been performed; animal studies show placental transfer.

Precautions & Warnings

Use caution in hepatic impairment. In heart failure, monitor clinical status for 2-3 hours after initiation and after each dose increase.
