Overview

Flonaspray 50 mcg/spray Nasal Spray contains Fluticasone Propionate, a synthetic trifluorinated corticosteroid with potent topical anti-inflammatory activity. Following nasal administration, Fluticasone Propionate produces anti-inflammatory and vasoconstrictor effects by reducing mediator cell infiltration at the epithelial level, inhibiting capillary dilation and permeability, and stabilizing lysosomal membranes. It is indicated for the prophylaxis and treatment of seasonal and perennial allergic rhinitis and for the prevention of recurrence of nasal polyps following surgical removal.

Dosage and Administration

Adults: 2 sprays in each nostril once daily, preferably in the morning. If required, 2 sprays in each nostril twice daily may be used. Total daily dose should not generally exceed 200 mcg (4 sprays). Children 4–11 years: 1 spray in each nostril once daily; in some cases 1 spray twice daily. Total daily dose should not exceed 100 mcg (2 sprays). Children below 4 years: Safety and effectiveness have not been established. Use regularly as directed, as effectiveness depends on consistent use. Prime the pump before first use or if not used for a week or more.

Interaction

At recommended nasal doses, Flonaspray has minimal systemic absorption. However, potent CYP3A4 inhibitors such as ritonavir and ketoconazole may increase systemic exposure to Fluticasone Propionate. Concurrent use with such agents should be approached with caution.

Contraindications

Flonaspray is contraindicated in patients with known hypersensitivity to Fluticasone Propionate or any of the excipients in the formulation.

Side Effects

Most common: drying and irritation of the nose and throat, unpleasant taste or smell, and increased incidence of nosebleeds (epistaxis). Rare: nasal septum perforation, wheezing, cataracts, glaucoma, and increased intraocular pressure. Although systemic effects are minimal at recommended doses, the risk increases with doses above those recommended.

Pregnancy and Lactation

There are no adequate and well-controlled studies in pregnant women. Flonaspray should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether Fluticasone Propionate is excreted in human breast milk; however, other corticosteroids are excreted in breast milk. Caution should be exercised when administering Flonaspray to nursing mothers.

Precautions and Warnings

Larger than recommended doses should be avoided as they increase the risk of systemic corticosteroid effects. Rare instances of nasal septum perforation, cataracts, glaucoma, and increased intraocular pressure have been reported following intranasal corticosteroid use. If these occur, discontinue use and consult a physician. Keep all medicines out of reach of children.
