Mechanism of Action
Adapalene is a third-generation synthetic retinoid that selectively binds retinoic acid receptors RAR-β and RAR-γ in the cell nucleus of keratinocytes. This normalises abnormal follicular keratinisation — the primary event in comedone formation — and reduces follicular plugging. It also suppresses inflammatory mediators (arachidonic acid metabolites) involved in inflammatory acne lesions. Unlike first-generation retinoids, adapalene has a more stable chemical structure with lower irritation potential, and as a cream formulation it is particularly suited to patients with dry or sensitive skin.
Relevance to Diabetes
Acne is more prevalent in patients with insulin resistance, hyperinsulinaemia, Type 2 Diabetes, and PCOS — due to IGF-1 and androgen-driven sebaceous gland overactivity. Effective topical retinoid therapy reduces comedonal and inflammatory lesions, lowering the risk of secondary skin infections which are disproportionately serious in diabetic patients due to impaired wound healing and immune function.
Indications
- Mild to moderate acne vulgaris (comedonal and mild inflammatory)
- Maintenance therapy after acne has been controlled
- Acne in patients with dry or sensitive skin (cream is better tolerated than gel)
- Acne associated with insulin resistance or PCOS
Dosage and Administration
Dose: Apply a thin layer to affected areas once daily in the evening.
- Wash face gently and pat dry (wait 20–30 minutes if skin is irritated)
- Apply pea-sized amount; avoid eyes, lips, nostrils, and mucous membranes
- Apply broad-spectrum SPF 30+ sunscreen every morning without fail
- Allow to absorb before applying moisturiser if needed
Duration: Improvement expected from 8–12 weeks. Maintenance use may continue indefinitely under medical supervision.
Warnings
- Initial skin purging (worsening) in first 2–4 weeks is expected — do not stop treatment
- Increases photosensitivity significantly — daily sunscreen is mandatory
- Avoid in pregnancy (teratogenic potential of retinoids)
- Do not apply to eczematous, sunburned, or broken skin
- Avoid concurrent use with harsh cleansers, alcohol-based products, or other exfoliants

- Hypersensitivity to adapalene or any retinoid
- Pregnancy
- Breastfeeding (avoid application near breast)

- Skin dryness, scaling, erythema — very common in first month
