Overview
Anleptic CR 200 mg Tablet is a controlled-release brand of Carbamazepine manufactured by Square Pharmaceuticals PLC. Carbamazepine depresses activity in the nucleus ventralis of the thalamus and reduces synaptic propagation of excitatory impulses by limiting the influx of sodium ions across cell membranes, helping to control seizures and stabilize mood and nerve pain conditions.
Indications
Anleptic CR is indicated for:
- Partial and secondary generalized tonic-clonic seizures
- Primary generalized tonic-clonic seizures
- Trigeminal neuralgia
- Prophylaxis of bipolar disorder
Pharmacology
Carbamazepine depresses activity in the nucleus ventralis of the thalamus, reduces synaptic propagation of excitatory impulses, or decreases summation of temporal stimulation leading to neural discharge by limiting influx of sodium ions across the cell membrane, or by other unknown mechanisms. It also stimulates the release of antidiuretic hormone (ADH) and potentiates its action in promoting reabsorption of water.

Carbamazepine may be used alone or in combination with other anticonvulsants. The tablets can be taken with or without regard to meals.
Interaction
Galactorrhoea has been reported in a few women on oral contraceptives within the first two months of Anleptic CR treatment. Hepatic enzyme inducers such as phenytoin may increase the metabolism of carbamazepine, requiring a dosage increase.
Contraindications
Anleptic CR should not be used in patients allergic to any of its ingredients, or in those with atrioventricular block, a history of bone marrow depression, hereditary porphyrias, allergy to tricyclic antidepressants, or in patients who have taken a monoamine-oxidase inhibitor (MAOI) within the last 14 days.
Side Effects
Common side effects include dizziness, drowsiness, ataxia, dry mouth, abdominal pain, nausea, vomiting, anorexia, leucopenia, proteinuria, bradycardia, heart failure, and hypotension. Erythematous skin rash and aplastic anemia may also occur. The most severe adverse reactions involve the hemopoietic system, skin, and cardiovascular system. This medicine may increase sensitivity to sunlight.
Pregnancy and Lactation
Pregnancy Category D. Carbamazepine and its epoxide metabolite are transferred to breast milk. Because of the potential for serious side effects, a decision should be made whether to discontinue nursing or discontinue the drug.
Precautions and Warnings
This medicine may cause dizziness and drowsiness; special care should be taken while driving or operating machinery. It may cause skin reactions — any rash, skin peeling, or unexplained skin reaction should be reported to a doctor immediately. It may rarely cause liver problems, requiring consultation if unexplained itching, jaundice, dark urine, nausea, vomiting, abdominal pain, or loss of appetite occur.
Anleptic CR decreases blood levels of hormonal contraceptives containing estrogen and/or progesterone, which may make them less effective or cause breakthrough bleeding. Women requiring contraception while on this medicine should use a contraceptive containing at least 50 micrograms of oestrogen or a non-hormonal method.
Treatment should not be stopped suddenly unless advised by a doctor, as abrupt discontinuation may cause symptoms to return. Caution is advised in patients with mixed seizures including absence seizures, elderly patients, and those with a history of heart, kidney, liver, or psychotic illness, raised intraocular pressure (e.g. glaucoma), blood disorders caused by other medications, or previous interrupted Anleptic CR therapy due to side effects or allergy.
Use in Special Populations
Renal impairment: Caution advised in patients with a history of kidney disease.
Hepatic impairment: Caution advised in patients with a history of liver disease; liver function should be monitored.
