Paracetamol is a centrally acting analgesic and antipyretic. It inhibits prostaglandin synthesis in the CNS and reduces fever by acting on the hypothalamic heat-regulating center. Unlike NSAIDs, it does not irritate the gastric mucosa. The 1000 mg strength provides stronger analgesic coverage for moderate pain and is suitable for adults who require higher-dose paracetamol therapy.
Indications
- Relief of mild to moderate pain: headache, toothache, backache, rheumatic and muscle pain
- Reduction of fever in common cold, influenza, and other febrile conditions
- Post-operative pain management as part of multimodal analgesia
- Suitable for patients who cannot tolerate aspirin or NSAIDs
Dosage & Administration
Adults and children over 12 years: 500 mg–1000 mg every 4–6 hours as needed. Maximum 4000 mg per 24 hours. Elderly: reduce frequency in renal/hepatic impairment.
Contraindications & Precautions
- Hypersensitivity to paracetamol or any excipient
- Severe hepatic impairment or active liver disease
- Caution in chronic alcohol use (>3 alcoholic drinks/day)
- Avoid concurrent use with other paracetamol-containing products
- Overdose may cause fatal hepatic necrosis — do not exceed recommended dose
Side Effects
Well tolerated at recommended doses. Rare: skin rash, urticaria, angioedema, blood dyscrasias (thrombocytopenia, leukopenia). Hepatotoxicity occurs with overdose.
Drug Interactions
Warfarin: enhances anticoagulant effect with regular use — monitor INR. Isoniazid, rifampicin, carbamazepine, alcohol: increase hepatotoxic risk. Probenecid: raises paracetamol levels. Cholestyramine: reduces absorption.
