What is Vibrenta?

Vibrenta is Incepta Pharmaceuticals' brand of insulin glargine, a long-acting recombinant human insulin analogue given once daily to manage blood glucose in adults and children with type 1 diabetes and in adults with type 2 diabetes. It is produced by recombinant DNA technology using a non-pathogenic laboratory strain of Escherichia coli (K12). • Switching from once-daily NPH: same unit dose. Switching from twice-daily NPH: 80% of total NPH dose.
• Rotate injection sites within the same body region (abdomen, thigh, or deltoid).
• Vial: disinfect the rubber stopper, draw air equal to the dose into the syringe, inject air into the vial, invert and draw the correct dose, expel air, and inject immediately.

Side Effects

Common: hypoglycemia, injection-site reactions (redness, swelling, itching). Uncommon: lipodystrophy at injection sites with poor site rotation, pruritus, rash. Rare: generalized hypersensitivity reactions.

Drug Interactions

Enhance hypoglycemia: oral antidiabetic drugs, ACE inhibitors, disopyramide, fibrates, fluoxetine, MAO inhibitors, propoxyphene, pentoxifylline, salicylates, sulfonamide antibiotics. Reduce insulin effect (raise glucose): thiazides, glucocorticoids, thyroid hormones, beta-sympathomimetics, growth hormone, danazol. Mask or alter hypoglycemia response: beta-blockers, clonidine, lithium salts, alcohol (may either potentiate or weaken the glucose-lowering effect).

Contraindications

• Active hypoglycemia.
• Known hypersensitivity to insulin glargine or any excipient.
• Intravenous use, or use in subcutaneous insulin infusion pumps.

Pregnancy and Lactation

Pregnancy: use only if the potential benefit justifies the potential risk to the fetus (consult a physician). Lactation: unknown whether excreted in human milk; caution advised, and nursing mothers may need adjustment of insulin dose and diet.
