Overview

Diltizem SR 90 mg Tablet contains Diltiazem Hydrochloride in a sustained-release formulation, manufactured by Square Pharmaceuticals PLC. Diltiazem is a calcium channel blocker that reduces heart rate and dilates coronary and peripheral arteries. It is used for chronic stable angina pectoris, variant (Prinzmetal's) angina, hypertension, and control of ventricular rate in atrial fibrillation/flutter.

Dosage and Administration

Adults: 90 mg SR twice daily, adjusted as needed; up to 360 mg/day in divided doses as prescribed. Swallow tablets whole. Take with or without food consistently.

Interaction

Increases plasma levels of digoxin, cyclosporin, carbamazepine, and statins (especially simvastatin). Additive bradycardia with beta-blockers and digoxin. Avoid with IV beta-blockers. Cimetidine and erythromycin increase diltiazem levels. Rifampicin reduces levels.

Contraindications

Contraindicated in severe bradycardia, second- or third-degree AV block (without pacemaker), sick sinus syndrome, severe hypotension, acute MI with pulmonary congestion, and known hypersensitivity.

Side Effects

Bradycardia, first-degree AV block, headache, oedema, dizziness, fatigue, and constipation. Rarely, hepatotoxicity.

Pregnancy and Lactation

Use with caution in pregnancy; safety not fully established. Excreted in breast milk — use with caution during breastfeeding.

Precautions and Warnings

Monitor heart rate and blood pressure, especially when combined with other rate-limiting drugs. Do not crush or break SR tablets. Avoid abrupt withdrawal. Monitor liver function with prolonged use.

Storage Conditions

Store below 30°C in a cool, dry place. Protect from moisture and light.

Frequently Asked Questions

Can diltiazem SR tablets be cut in half?
No — sustained-release tablets should be swallowed whole; cutting or crushing destroys the extended-release mechanism and may cause an excessive rapid dose release.

What is the advantage of SR diltiazem over immediate-release?
The SR (sustained-release) formulation provides more stable blood levels throughout the day, allowing twice-daily rather than three-to-four times daily dosing, improving patient compliance and avoiding peak-related side effects.
