Emjard M 5/500 mg Tablet

Overview

Emjard M 5 mg+500 mg Tablet contains Empagliflozin 5 mg and Metformin Hydrochloride 500 mg, manufactured by Square Pharmaceuticals PLC. This fixed-dose combination provides dual antidiabetic action: Empagliflozin inhibits renal SGLT-2 to promote urinary glucose excretion, while Metformin reduces hepatic glucose production and improves insulin sensitivity. The combination is used for type 2 diabetes mellitus in patients inadequately controlled on either agent alone, or to simplify dual therapy.

Dosage and Administration

Adults: 1 tablet twice daily with meals. Individualise dose based on current antidiabetic regimen. Do not crush or chew if modified-release formulation. Swallow whole with adequate water.

Interaction

As per individual components. Insulin/secretagogue combinations increase hypoglycaemia risk. Iodinated contrast media — temporarily discontinue metformin before procedures involving contrast. NSAIDs and alcohol increase risk of lactic acidosis (metformin). Diuretics may cause volume depletion (empagliflozin).

Contraindications

Contraindicated in type 1 diabetes, DKA, eGFR <45 ml/min/1.73m², severe hepatic impairment, active/unstable heart failure with shock, contrast media use (temporary hold), excessive alcohol use, and known hypersensitivity to either component.

Side Effects

Combined effects of both agents: GI disturbances (metformin — especially initially), genitourinary infections and increased urination (empagliflozin), volume depletion, and rarely lactic acidosis (metformin).

Pregnancy and Lactation

Not recommended during pregnancy or breastfeeding for either component.

Precautions and Warnings

Temporarily stop if hospitalised or before surgical procedures. Ensure adequate hydration. Monitor renal function annually and before dose changes. Take with meals to reduce GI side effects of metformin.
