Overview
Facticin 320 mg Tablet contains Gemifloxacin Mesylate, a fourth-generation fluoroquinolone antibiotic manufactured by Square Pharmaceuticals PLC. Gemifloxacin has broad-spectrum bactericidal activity by inhibiting bacterial DNA gyrase and topoisomerase IV, essential enzymes for DNA replication. It has enhanced activity against Streptococcus pneumoniae (including drug-resistant strains) compared to earlier fluoroquinolones. Clinical uses include community-acquired pneumonia (CAP), acute bacterial exacerbations of chronic bronchitis (ABECB), and atypical respiratory pathogens.
Dosage and Administration
Adults: 320 mg once daily for 5–7 days (CAP) or 5 days (ABECB), taken with or without food. Swallow whole with adequate water. Take at the same time each day.
Interaction
Antacids, iron, zinc, and sucralfate reduce absorption — take gemifloxacin 2 hours before or 4 hours after these products. QT-prolonging drugs: additive QT interval prolongation risk — avoid co-administration with amiodarone, quinidine, sotalol. NSAIDs may increase risk of CNS side effects. Warfarin: may increase INR — monitor.
Contraindications
Contraindicated in known hypersensitivity to gemifloxacin or any fluoroquinolone, history of prolonged QT interval, uncorrected electrolyte disturbances, and concurrent use with QT-prolonging drugs. Avoid in patients with a history of tendon disorders related to fluoroquinolone use.
Side Effects
Diarrhoea, nausea, headache, and skin rash (risk higher in women under 40 and post-menopausal women on HRT). Photosensitivity. Tendon rupture (rare but serious — especially with concurrent corticosteroid use). QT prolongation. CNS effects: dizziness, insomnia, seizures (rare).
Pregnancy and Lactation
Avoid during pregnancy — fluoroquinolones have been shown to cause arthropathy in animal studies; potential risk to developing cartilage. Not recommended during breastfeeding.
Precautions and Warnings
Avoid excessive sun exposure — use sunscreen. Report tendon pain or swelling immediately and discontinue. Ensure adequate hydration. Not suitable for patients with tendinopathy or myasthenia gravis.
