Overview

Colimax 0.6 mg Tablet contains Colchicine, manufactured by Square Pharmaceuticals PLC. Colchicine inhibits microtubule polymerisation and neutrophil migration, reducing the inflammatory response in acute gout. It is used for the treatment and prevention of acute gouty arthritis attacks and for familial Mediterranean fever (FMF).

Dosage and Administration

Acute gout: 1.2 mg (two tablets) at onset, followed by 0.6 mg one hour later; then 0.6 mg twice daily for prophylaxis as prescribed. Doses must be adjusted in renal and hepatic impairment.

Interaction

Significant interaction with P-glycoprotein and CYP3A4 inhibitors (e.g. clarithromycin, cyclosporin, ketoconazole) — can increase colchicine toxicity severely. Statins: increased risk of myopathy. Avoid concurrent use with strong inhibitors.

Contraindications

Contraindicated in patients with severe renal or hepatic impairment, known hypersensitivity to colchicine, and concurrent use of strong P-gp or CYP3A4 inhibitors.

Side Effects

Diarrhoea, nausea, vomiting, and abdominal cramping are most common. Serious toxicity (bone marrow suppression, myopathy, neuropathy) may occur with overdose or drug interactions.

Pregnancy and Lactation

Contraindicated in pregnancy. Excreted in breast milk — not recommended during breastfeeding.

Precautions and Warnings

Use with care in elderly and renally impaired patients. Report immediately if unexplained muscle pain, weakness, or numbness develops. Complete blood count monitoring is advisable with long-term use.
