Overview

Emjard 10 mg Tablet contains Empagliflozin, a selective and reversible inhibitor of sodium-glucose cotransporter-2 (SGLT-2), manufactured by Square Pharmaceuticals PLC. SGLT-2 inhibitors reduce renal glucose reabsorption, leading to urinary glucose excretion (glucosuria) and thereby lowering blood glucose levels in a manner independent of insulin. Empagliflozin is used for type 2 diabetes mellitus management to improve glycaemic control, and has significant evidence for cardiovascular risk reduction and heart failure management in patients with or at risk of cardiovascular disease.

Dosage and Administration

Adults: 10 mg once daily, taken in the morning, with or without food. Dose may be increased to 25 mg once daily if additional glycaemic control is needed and tolerated. Reduce dose or discontinue if eGFR falls below 45 ml/min/1.73m².

Interaction

Insulin and insulin secretagogues (sulfonylureas) — increased hypoglycaemia risk; dose reduction of the secretagogue may be necessary. Diuretics — increased diuretic effect and risk of volume depletion. Monitor renal function when used with NSAIDs.

Contraindications

Contraindicated in type 1 diabetes, diabetic ketoacidosis, eGFR <45 ml/min/1.73m² (for glucose lowering), end-stage renal disease, dialysis, and known hypersensitivity to empagliflozin.

Side Effects

Genitourinary infections (urinary tract infections, genital mycotic infections — thrush), increased urination, volume depletion/dehydration (especially in elderly), Fournier's gangrene (rare but serious). Risk of euglycaemic DKA (rare).

Pregnancy and Lactation

Not recommended during pregnancy — may harm the developing foetus, especially during the second and third trimesters. Not recommended during breastfeeding.

Precautions and Warnings

Maintain adequate hydration. Temporarily discontinue before planned surgery or during prolonged fasting (DKA risk). Seek urgent care for fever, nausea, abdominal pain, or difficulty breathing (possible DKA, even at near-normal glucose). Inspect genital area regularly.
