Description

Losart (Losartan Potassium) is the first orally active, long acting and specific inhibitor of Angiotensin II at its receptor site (type AT1). Angiotensin II is the primary mediator of renin-angiotensin-system and is involved in the blood pressure control at cellular, tissue and organ integration levels. Losartan and its principal active metabolite block the vasoconstrictor and aldosterone-secreting effects of

Angiotensin II by selectively blocking the binding of Angiotensin II to the AT1 receptor. Losart is suitable for the patients who are unable to tolerate ACE inhibitors because of persistent cough.

Composition

LOSART 25: Each tablet contains Losartan Potassium USP 25 mg.

LOSART 50: Each tablet contains Losartan Potassium USP 50 mg.

LOSART 100: Each tablet contains Losartan Potassium USP 100 mg.

Indications

Losart is indicated for the treatment of all grades of hypertension and congestive heart failure. It may be used alone or in combination with other anti-hypertensive agents.

Dosage & Administration

The usual dose is 50 mg once daily; if necessary increased after several weeks to 100 mg once daily. For elderly over 75 years, intravascular volume depletion and moderate to severe renal impairment; initially 25 mg once daily is suggested.

Contraindications

It is contraindicated in patients who are hypersensitive to Losartan potassium or any component of this product.

Precautions

Losartan should be used with caution in patients with known hypersensitivity to the drugs that act through renin-angiotensin system. Special precaution should be taken for patients with renal and hepatic impairment and a lower dose should be given. Safety and effectiveness of Losartan in paediatric patients have not been established.

Side Effects

The incidence of adverse effects of Losartan is low and comparable to placebo.

The most common adverse events occurring with Losartan at a rate of >1% above placebo were upper respiratory infection (7.9% vs 6.9%), insomnia (1.4% vs 0.6%) and dyspepsia (1.3% vs 1.2%). Side effects like asthenia/fatigue, abdominal pain, chest pain, nausea, headache, pharyngitis were also reported at a rate of 1%.

Angioedema including swelling of the larynx, face, lips and tongue has been reported rarely in patients treated with Losartan. The incidence of cough is similar to placebo.

Use in Pregnancy and Lactation

Losartan should not be used in pregnancy and it should be discontinued as soon as possible when pregnancy is detected. It should not be prescribed during lactation, as there is no information in human on the excretion of Losartan in breast milk.

Drug interactions

No drug interaction of clinical significance has been identified. Compounds which have been studied in clinical pharmacokinetic trials include hydrochlorothiazide, digoxin, warfarin, cimetidine, ketoconazole and phenobarbital.
