Dosage and Administration

Adults: 100-200 mg once or twice daily, depending on pain severity, not exceeding 400 mg per day. Capsules should be swallowed whole, not crushed or chewed, and may be taken with or without food.

Interaction

Concomitant use with CNS depressants (alcohol, sedatives, opioids), MAO inhibitors, and SSRIs/SNRIs increases risk of respiratory depression and serotonin syndrome. Carbamazepine reduces tramadol's analgesic effect.

Contraindications

Contraindicated in acute intoxication with alcohol, hypnotics, or other centrally acting analgesics; in patients with known hypersensitivity to tramadol; and in patients treated with MAO inhibitors.

Side Effects

Nausea, dizziness, constipation, drowsiness, dry mouth, and sweating. Respiratory depression and dependence may occur with prolonged or high-dose use.

Pregnancy and Lactation

Not recommended during pregnancy as Tramadol crosses the placenta; may cause neonatal withdrawal symptoms. Not recommended during breastfeeding.

Use with caution in patients with a history of seizures, respiratory depression, or substance abuse. Risk of dependence and tolerance with long-term use; dose should be tapered when discontinuing.
